Coordinated Care Conferences: keeping every part of your child’s care aligned

When multiple therapists, a school team, and a family are all involved in a child’s care, regular coordination is what keeps everyone working toward the same goals.

What are coordinated care conferences?

Coordinated care conferences are scheduled check-ins where your child’s care team — which may include ABA therapists, speech-language pathologists, occupational therapists, and clinical supervisors — reviews progress together and, where relevant, connects with your child’s school team as well. The goal is simple: make sure no part of your child’s care is operating in isolation.

Who attends a care conference

At minimum, these conferences include your family and your child’s lead clinical staff. Depending on your child’s plan, we also invite school staff, other treating providers, or case managers so that everyone with a role in your child’s development has current, accurate information rather than secondhand summaries.

What gets discussed

Progress toward each goal in your child’s Individual Treatment Program
Any changes needed to therapy hours, focus areas, or approach
Coordination with school-based plans, such as an IEP or 504 plan
Family questions, concerns, or shifting priorities at home
Next steps and timing for the following review

Why this coordination matters

Without regular coordination, it’s common for a child to be working on conflicting strategies at home, at school, and in therapy — or for progress made in one setting to go unnoticed in another. Coordinated care conferences close that gap, giving families one consistent, shared picture of how their child is doing.

How often conferences happen

The frequency of care conferences depends on your child’s specific plan, but most families can expect a formal review on a regular recurring schedule, with additional check-ins available any time a significant change or concern comes up between scheduled meetings.

Getting started

Coordinated care conferences are automatically included once your child begins services with a finalized Individual Treatment Program — there’s nothing separate to schedule on your end. Your care coordinator will reach out directly to set the first conference date and confirm which school staff or outside providers, if any, should be invited.

Common questions about care conferences

Can our child’s school be included even if they attend a different district? Yes. We coordinate with school staff regardless of district, as long as your family provides the appropriate consent for us to share information.
What if we can’t attend a scheduled conference? We work with families to reschedule around real conflicts, and can share a written summary of what was discussed if a live meeting isn’t possible.
