What is a coordinated care conference?

A coordinated care conference is a planning meeting where your child’s care team reviews their evaluation or treatment plan and coordinates services across every provider involved. It is a covered service under Minnesota’s EIDBI benefit, sometimes called an EIDBI care conference or a medical team conference.
Minnesota’s Department of Human Services describes it as a voluntary meeting between your child, your family, the EIDBI providers, and anyone else you request. DHS treats it as a best practice for children with complex needs because it connects EIDBI with related care such as speech therapy, occupational therapy, mental health services, and special education.
It is not a school IEP meeting and not a therapy session. Nobody is evaluating your child during it.

Is an autism care team meeting the same thing?

In everyday language, often yes. Under EIDBI, the formal conference comes with protections an informal autism care team meeting does not. Your family, as caregiver or legal guardian, must be included, and every decision is summarized in your child’s treatment plan and health record.
That record is what makes the conference useful months later. When a new therapist or teacher joins, they can see exactly what was agreed and why.

Who It’s For

Who is this service for?

Families of children receiving EIDBI services, or about to start, who want every provider working from the same plan. It is especially valuable when your child:
Receives more than one therapy, such as ABA plus speech or occupational therapy
Has an IEP or 504 plan at school
Is starting services for the first time after a CMDE
Is going through a transition, such as starting kindergarten or changing schools
Works with a county case manager or other outside providers

Benefits

Benefits of a coordinated care conference for your child and family

The biggest benefit is the consistency your child can feel. When home, school, and therapy use the same approach, skills practiced in one place carry over to the others.
One plan instead of several. Providers stop working from separate playbooks.
Your priorities become written goals. What matters at home, such as mealtimes or bedtime, is part of the plan.
Fewer mixed messages. Everyone hears the same update at the same time.
Earlier course corrections. Progress reviews catch what is not working before months pass.
Less running around. A conference can take place on the same day as other EIDBI services.
A record that follows your child. New staff and teachers can see exactly what was agreed.
In our experience, the most common frustration families describe before coming to us is getting conflicting updates from providers who never talk to each other. These conferences exist to end that.
How It Works

How it works, from first call to follow-up

01

You reach out

Call or message us. A member of our team typically responds within one to two business days.
02

Evaluation completed or reviewed

Your child’s evaluation is completed or reviewed. The first conference follows the CMDE.
03

First conference scheduled

Your care coordinator schedules it. We confirm with you which school staff or outside providers to invite and collect any consent needed.
04

The plan is built

Together we review the CMDE results, agree on goals, and assign clear roles.
05

Follow-ups keep it on track

They line up with your child’s treatment plan progress reviews, which DHS requires at least every six months, or sooner whenever a need comes up.

School Coordination

How does IEP therapy coordination work?

Your child can have two plans at once: an IEP from school and a treatment plan from EIDBI. IEP therapy coordination keeps them pulling in the same direction. DHS explains that EIDBI covers medically necessary services beyond what the school district provides, so the two plans should complement each other, not repeat each other.
Good IEP therapy coordination means everyone uses the same words, visuals or communication system for the same skill, and each plan covers what its setting does best. It is the kind of school and therapy coordination autism programs should build in from the first meeting.
DHS and the Minnesota Department of Education have published a joint memo on how EIDBI providers and school districts can collaborate, and the state offers a comparison of special education and EIDBI for families who want the full detail.

Your top three priorities for daily life at home

Questions worth asking

Which goals matter most over the next six months, and why?
How will I know a goal is being met?
What can I practice at home between sessions?
How will school staff and your team share updates?
Common Questions

What if you cannot attend, or your child’s school is in another district?

Neither one is a barrier. We reschedule around real conflicts, and if a live meeting is not possible, we share a written summary of what was discussed. When it is medically appropriate, the conference can also take place by video.
We coordinate with school staff regardless of district, as long as your family consents to us sharing information. That makes school and therapy coordination autism families need possible even when home, school and therapy are in different cities.
Cost & Coverage

Does a coordinated care conference cost anything?

It is a covered EIDBI service, and for fee-for-service Medical Assistance it does not require prior authorization, so it never holds up the start of care. Managed care and private plans can set their own rules.
An EIDBI care conference is handled like other covered EIDBI services. Our team verifies your specific benefits before services begin and explains any out-of-pocket responsibility clearly. See our EIDBI insurance and payment options.
